Don't think anyone's made one before. I can't even find a name for it in the EU 
Would probably be a good EVA foam build if the 3D model was available so one could make a pepakura template. It actually has a lot of similarities to the Iron Man helmets, which are very popular EVA builds. Same sort of segmented stripe construction where everything is just half-circle lanes connected together. Without a model though, it'd probably be easier to just custom sculpt it. Wouldn't be too hard considering it's mostly just a dome, very expensive for a one-off though.

I've been working on mine for a while now just never thought anyone else had an interest in this... If it turns up decent I might do a run

I believe it's mainly seen in the Season 4.20 Episode Bounty if you wanna try to fish for screens.
A few other eps are mentioned too here http://clonewars.wikia.com/wiki/Boba_Fett, but iirc he didn't have it in the earlier seasons.
Other than that, it is very similar to the Ketsu Onyo helm as @haujohn mentioned, and that one appears to just be a complete dome. I'd figure Boba's is the same since they kept the designs pretty simple in the show.

Does anyone here know where I can find a clone wars era captain rex rangefinder. I have looked on amazon, eBay, and facebook marketplace and have not found just the rangefinder.
