What Fett Helmet is this

Can anyone say what Fett Helmet this may have come from ? I found it on Ebay quite awhile back and just opened the box yesterday

It looks like a fan sculpt made using the WoF templates. I think I've seen it before.

That could be , Also I just noticed signs of where the dent used to be before the helmet was molded .
