Borden Connector- Ordering Problems?

Has anyone tried ordering a Borden Connector from http://www.dentalsuppliersofamerica....l?pc=6&pid=123 ?

I tried ordering one Friday, and it said they'd email me some kind of form I had to fill out before the order would be processed. It hasn't arrived yet and I'm wondering if that's normal?

Is there another place where I can order them without all the hassle?
